Request we do not use or share certain health information for treatment or payment.

  • To treat you: Partnering with other professionals who are treating you.

  • To run our practice: Using and sharing info to improve care and contact you.

  • To bill for services: Getting payment from health plans or other entities.

Colorado Specific Mandates

Under Colorado law, we must breach confidentiality without authorization in these cases:

1.

Suspected child or elder abuse/neglect.

2.

Serious danger of violence to others.

3.

Immediate danger to self / self-neglect.

4.

Under legal subpoena by a court.
